Report Requests

CPHP provides 3 main types of reports.

1. CPHP provides comprehensive written reports to the Colorado Board of Medical Examiners (BME) per the requirements of a Formal Referral.
2. CPHP provides workplace reports when a client has been formally referred to CPHP. These are intended to inform the workplace of CPHP’s evaluation process, compliance with the evaluation, and recommendations that may emanate from the evaluation.
3. CPHP provides credentialing reports at the request of a client or the credentialing entity. The initial report is free of charge and there is a $35.00 cost for subsequent reports. Please complete our Report Request Form if you would like to request a credentialing form be generated.

Other points to remember:

• CPHP does not provide reports to clients whose cases have been inactivated for more than one year.
• Please contact your Clinician regarding reports that do not pertain to a Colorado medical license or Colorado Physician Assistant license as there is a separate fee structure for these reports.
• Please contact a Clinician for more information about any of the report policies of CPHP.
